How do I fax in my Refill order?

1. Pick the appropriate form (see forms below), and print it. (Which form you fill out depends on if you are refilling an order or refilling one with a new prescription).
2. Fill out the appropriate form completely (Remember to print clearly.)
3. After you’ve completed the “Refill Order Form”, fax it to us, along with any new prescriptions you might have. Fax us free of charge at 1-866-535-0563 (toll free)
