1. Depending on if you’re a “New” or “Existing” customer, we ask you to print one of the following forms. These forms ensure we receive the correct information regarding if you are a new or returning patient, and may help us with drug interactions and side effects if you are already in our system.
2. Fill out the appropriate form, remembering to print clearly. This is to ensure our pharmacy staff can correctly identify you based on the information you filled out.
